Client Therapy Agreement

Hypnotherapy by Madi 
Effective Date: 28.4.2025 

This Client Therapy Agreement ("Agreement") is between Hypnotherapy by Madi ("Therapist") and the undersigned ("Client"). 
It outlines the understanding between Therapist and Client regarding hypnotherapy services. 

 

1. Purpose of Therapy 

Hypnotherapy is a collaborative process focused on personal development, empowerment, wellness, and addressing specific goals. 
It is not a substitute for medical, psychological, or psychiatric treatment. 

You agree to use hypnotherapy services responsibly, acknowledging that individual results vary and cannot be guaranteed. 

 

2. Client–Therapist Relationship 

  • The relationship between Client and Therapist is professional and therapeutic only. 

  • The Therapist’s role is to facilitate, support, and guide you through the hypnotherapy process. 

  • Personal or social relationships outside of therapy are not permitted to protect the integrity of the therapeutic work. 

 

3. Confidentiality 

  • All sessions and client records are confidential. 

  • Information will only be shared with Client’s written consent or if required by law (e.g., risk of harm to self or others, abuse disclosure, or court order). 

  • In the case of clients under 18 years old, relevant information may be shared with parents/guardians as needed while maintaining appropriate confidentiality.

4. Sessions 

  • Sessions last approximately 50-60 minutes, unless otherwise agreed. 

  • Smoking/vaping cessation sessions last approximately 90-120 minutes, unless otherwise agreed. 

  • Appointments are scheduled in advance. 

  • Clients are expected to arrive promptly; sessions cannot be extended if you arrive late. 

  • Sessions take place online only, using Google Meets as the platform for sessions. 

 

5. Missed Sessions and Cancellations 

  • Cancellation Policy: At least 24 hours’ notice is required for cancellations or rescheduling. 

  • Late cancellations (less than 24 hours' notice) or no-shows may be charged 50% of the session fee or the full fee, at Therapist’s discretion. 

  • In cases of emergency, exceptions may be made. 

Cancellations should be made via email or phone (hypnotherapybymadi@outlook.com, 07824743540). 

 

6. Fees and Payment 

  • Session fees are £80 per session if choosing pay per session, or you can buy a block of 4 sessions at £300. 

  • Smoking/vaping cessation session fee is £250, for a one-off session. 

  • Payment is required at least 48 hours before the session. 

  • Accepted payment methods: bank transfer. 

  • Missed sessions without notice may be charged the full session fee. 

Fee changes will be communicated in writing at least 30 days in advance. 

 

7. Misunderstandings 

If you have concerns about any aspect of your sessions or the therapy process, you are encouraged to discuss them with the Therapist. 
Clear communication is key to resolving misunderstandings quickly and respectfully. 

 

8. Ending the Therapeutic Relationship 

  • You may end therapy at any time. 

  • The Therapist may also conclude the therapeutic relationship if: 

  • The therapy is no longer beneficial or appropriate. 

  • The therapeutic relationship has been compromised. 

  • Fees are unpaid. 

  • Whenever possible, advance notice will be given, and referrals to other professionals will be offered if needed. 

 

9. Consent 

By signing this Agreement, you acknowledge that you: 

  • Understand the nature of hypnotherapy services. 

  • Have had the opportunity to ask questions and have them answered. 

  • Consent to participate voluntarily in therapy under the terms outlined.
